Patient Centered Medical Home Conference
HSRHA was pleased to partner with the Area Health Education Center to cosponsor a Patient Centered Medical Home Conference on April 14, 2012. The presentations and videos of most sessions can be viewed at www.ahec.hawaii.edu.

HSRHA also held its general membership meeting at 8-8:45 am on April 14, prior to the conference, at the same location and elected the 2012 Board of Directors.
The 2011 annual conference with a theme of Rural Communities Building Healthcare Teams was held April 5, 2011 at the Maui Beach Hotel.
The conference program contains the flow for the day which included panels and “rapid resource sharing sessions” for the 100 participants. Many of the presenters used slide shows in their presentation and have made them available below.
